These terms govern your membership and use of The Forum, located at 1271 N Steamboat Dr, Fayetteville, AR 72704. By joining or entering the facility, you agree to the terms below. Please read them carefully.

1. Membership

Memberships are month-to-month and billed on a recurring basis until cancelled. There is no long-term contract and no initiation fee. Rates, passes, and available plans are those listed at the time of purchase and may change with reasonable notice. Day passes and multi-day passes are valid only for the period stated and are non-refundable.

2. Access & Conduct

Members receive keyed access to the facility 24 hours a day. Your access credentials are personal to you and may not be shared, loaned, or used to admit non-members. To keep the space safe and welcoming for everyone, members agree to:

Rerack weights and wipe down equipment after use.
Treat other members, staff, and equipment with respect.
Follow posted safety rules and staff instructions.

We reserve the right to suspend or terminate access for conduct that endangers others, damages property, or violates these terms.

3. Billing & Cancellation

Recurring memberships continue until you cancel. You may cancel at any time; cancellation stops future billing and does not refund charges already processed. Please allow for your current billing period to complete. If a payment fails, we may suspend access until the balance is resolved.

4. Assumption of Risk

Physical training carries inherent risks. By using the facility and equipment, you acknowledge those risks and agree that you participate at your own risk. We recommend consulting a physician before beginning any new exercise program. The Forum is not liable for injury, loss, or damage except to the extent required by law.

5. Personal Property

The Forum is not responsible for lost, stolen, or damaged personal property. Please keep your belongings with you or otherwise secured. Items left behind may be donated or discarded after a reasonable period.
